Advance Care Planning (ACP) is a process that supports an individual’s understanding and sharing of personal values, life goals, and preferences related to future medical care, so that they get care that is consistent with these choices. While ACP is important for all populations, it is especially important for those whose life (and health) experiences fall outside of traditional, western, heteronormative contexts (i.e., those belonging to racial, ethnic, sexual, and/or gender minority communities).

The Diversity Access Team is assessing, tailoring, implementing, and evaluating ACP tools aimed at minority populations. For more information about our project, please visit
