Name, address, telephone number, e-mail address, student number, Social Insurance Number, educational history, financial history, and employment history.
Personal information is collected by the University because it relates directly to and is necessary for the operation of the Centre for Students with Disabilities (CSD). The CSD uses personal information collected about service providers in the selection of the providers, for the payment of providers and for following-up and monitoring service delivery. Personal information is disclosed only for the purpose for which it was obtained or compiled or for a use consistent with that purpose, including the delivery of an integrated student disability program.
Individuals and university offices to which personal information may be disclosed in the pursuit of the University's delivery of an integrated student disabilities program may include students with disabilities who work with or benefit from service providers and Accounts Payable.
Professional and non-professional service providers contracted by the Centre for Students with Disabilities.
